Support Coordination

At Linking the Next Steps VIC, our Support Coordination service helps you make the most of your NDIS plan. We understand that navigating the system can feel overwhelming, so we’re here to guide you — step by step — to connect with the right supports and build your confidence in managing them.

 

What we do:

Our experienced Support Coordinators help you:

  • Understand your NDIS plan and funding categories

  • Find and connect with the right providers and community supports

  • Coordinate multiple services so they work well together

  • Build your skills and independence in managing your supports

  • Advocate for your needs and assist with plan reviews or changes

 

Who it’s for:

Support Coordination is suitable for participants who:

  • Have multiple supports or complex needs

  • Are new to the NDIS and want help implementing their plan

  • Need guidance to connect with services, manage changes, or plan ahead

 

We focus on you — your strengths, goals, and preferences — so you can achieve the outcomes that matter most in your life.

Psychosocial Recovery Coaching

Supporting recovery, resilience, and wellbeing

 

Psychosocial Recovery Coaching (PRC) is designed for participants living with a psychosocial disability — a disability that arises from a mental health condition and affects how a person functions in everyday life.

 

What is a Psychosocial Disability?

According to the NDIS (2025–2026), a psychosocial disability occurs when a person experiences the ongoing effects of a mental health condition that make it difficult to do everyday things — like managing daily routines, socialising, studying, or working.  Not everyone with a mental health condition has a psychosocial disability. It depends on the functional impact — how much the condition affects daily life and participation over time.

 

The NDIS supports people with psychosocial disabilities to improve their independence, wellbeing, and community participation, through recovery-focused supports and services.

What we do:

Our Psychosocial Recovery Coaches work alongside you to:

  • Identify your personal recovery goals and strengths

  • Build your confidence, resilience, and decision-making skills

  • Connect you with NDIS-funded, community, and mental health supports

  • Develop a personalised recovery plan

  • Support you through challenges and celebrate your progress

 

Who it’s for:

Recovery Coaching is suitable for participants who:

  • Live with a psychosocial disability

  • Want support that focuses on recovery, not just coordination

  • Benefit from having a coach who understands mental health and capacity-building

 

This support is funded under the Capacity Building – Support Coordination and Psychosocial Recovery Coaching category in your NDIS plan.

Can I Have Both?

According to the NDIS 2025–2026 guidance, most participants will have either a Support Coordinator or a Psychosocial Recovery Coach, not both — because both roles include elements of coordination and capacity building.

 

However, the NDIA may fund both in limited circumstances where:

  • There is a clear and distinct need for both roles

  • The responsibilities of each role are well-defined and not duplicated

  • The combination provides better outcomes for the participant

 

If you have complex needs that include both psychosocial and other disabilities, we can help you discuss this with your planner or Local Area Coordinator to find the right balance of supports.
